I'm not going to pitch against agencies. I'll just tell you what happened when I used them.

When I ran Canova Medical, I hired marketing agencies. Some were good at talking. None of them understood what made a patient book a consultation — or what made them leave the page in ten seconds. Here's the honest version of why that matters for your clinic.

To be fair to agencies. If you have a large marketing budget, need multiple channels managed simultaneously at scale, or require a full creative team, an agency may be the right choice. I founded Canova Medical in Wilmslow in 2004. I ran it for fifteen years, grew it, and sold it in 2019. During that time I was the person being pitched to by agencies, signing the retainer agreements, reading the monthly reports, and wondering why the phone wasn't ringing more.

An agency is a team working across many clients and industries. Your account is typically sold by a senior person and managed by someone more junior. A specialist consultant works directly with you, has deep expertise in your specific sector, and has a client list small enough to give each one proper attention. For an independent aesthetic clinic, the difference in outcomes tends to be significant.
Marketing aesthetic medicine requires understanding patient psychology around appearance, UK ASA regulations for cosmetic advertising, the specific trust factors that make someone choose one practitioner over another, and the patient journey from first consideration to booked consultation. A generalist agency learns this second-hand from your brief. Someone who has run an aesthetic clinic already knows it — and those two positions are not equivalent.
I charge a monthly retainer. For SEO and campaign management combined, a realistic range for an independent UK aesthetic clinic is £1,200 to £2,000 per month — not including ad spend. This is often comparable to or less than agency fees, without the overhead of account managers, reporting layers, and onboarding processes that agencies build their pricing around. I'll give you a specific figure once I understand what you actually need.
Honestly, no. Three things. Do they understand the aesthetic sector specifically — not just marketing in general? Will you work directly with the person who has that expertise, or will your account be passed to someone junior? And do they measure success the way you do — by bookings and revenue — rather than by reach and impressions that don't pay your rent?
